Required Consent Form

All Shakopee Public Schools staff, students and family members are welcome to take advantage of the 2025-26 Fall Flu Shot Clinics led by Homeland Health Specialists.

Interested persons can sign up to receive the annual influenza vaccine and the Covid-19 vaccine at the community clinics noted on this page. Shakopee School District employees can receive their vaccines at designated employee clinics and will receive registration details in the Shakopee Staff Resources newsletter.

All persons should bring their insurance card. If you do not have insurance, free shots are available for children aged 6 months to 18 years of age.
Four (4) vaccine options will be available:

2025-26 trivalent influenza vaccine
FluMist (limited supply)
Senior Dose (limited supply)
COVID-19

September 25, 2025: Sun Path Elementary (lower lot, activities entrance)
October 6, 2025: West Middle School (main entrance)
October 7, 2025: Red Oak Elementary (main entrance)

What insurance plans are accepted?

If I don't have insurance, how much does a vaccine cost?

All persons should bring their insurance card. If you do not have insurance, free shots are available for children aged 18 months to 18 years of age. Adults without insurance can purchase a vaccination for $45, FluMist (if available) for $50 or Senior Dose (if available) for $75. The COVID vaccine is $190.
Cash or check is requested, and directly handled with Homeland Health Specialists).
*Offering the Trivalent Flu Vaccine for the best protection possible.

How do I sign up?

Step One: Register for any of the flu shot clinic dates below by selecting the appropriate sign-up link. First/last name and an email are required to register. Multiple appointments can be secured under one name. Appointments generally last 15 minutes. Please arrive five minutes prior to your scheduled time.

Step Two: Print and complete a consent form for each family member that will attend the noted clinic. Bring the form(s) with you to the clinic. A legal parent/guardian MUST sign a consent form for their child (if under the age of 18). For high school students signing up for a Shakopee High School appointment, parents/guardians do not need to be present.
Reminders: Please wear a short sleeve shirt and bring your insurance card, if applicable.
