General Intake: 952-496-8556Immunizations: 952-496-8407 (immunization clinics are offered on Monday afternoons from 1 to 5 p.m. at 792 Canterbury Road South in Suite A160. Appointments are necessary. No clinics on legal holidays. The clinic serves uninsured children and adults as well as those individuals who have out-of-pocket expenses for vaccines).Mobile Clinics: Rotating locations in Jordan (Radermacher Fresh Market), Savage (Scott County Library) and Shakopee (Slavic Baptist Church, 1205 10th Avenue AND Valley Green Workforce Center, 792 Canterbury Road South in Suite B231). The mobile clinics provide care for short term and ongoing health concerns; health screenings (cholesterol tests, blood pressure and blood glucose tests); assistance with obtaining affordable prescription medication; and information/referral to affordable health care and other community resources. Services are FREE for uninsured and underinsured residents of Scott County. St. Mary's Clinic
St. Mary's clinic provides health care services to low income, uninsured individuals and families in
the St Paul/Minneapolis metropolitan area.
